Soccer Camp

Trinity Canadian Reformed Church is once again running their Soccer Camp this summer! We are so excited and invite you to join us as we learn new soccer skills and/or strengthen existing ones, while learning more about how God works His redeeming grace in the story of Jonah, and calls his people to repent and trust in Him.

Our focus this year will be on the following Bible text: “Great is the Lord and most worthy of praise; his greatness no one can fathom. One generation will commend your works to another; they will tell of your mighty acts. They will speak of the glorious splendour of your majesty, and I will meditate on your wonderful works.” -Psalm 145:3-7

Soccer Camp is planned for July 8-12, 2019 and is for ages 4-11 (born in 2008-2015). There is no cost for your children to participate in this program!

Picture Consent

During your child’s week at Trinity Canadian Reformed Church’s Soccer Camp we may wish to take photographs of activities that involve your child. The photographs may be used for our bulletin board. Before taking any photographs of your child, we need your permission. Please select the appropriate box below. You can ask to see the images of your child that are taken and you may withdraw your consent at any time.

I understand that:
1. the Organizers may take images of activities that show the establishment and children in a positive light.
2. Photographers acting on behalf of the Church may take images for use in displays.

Medical Release Information

I give permission to administer EMERGENCY medical care for the child listed above when I cannot be reached at the time of an EMERGENCY. I have indicated our hospital of choice, doctor’s name and phone number as indicated below. Trinity Canadian Reformed Church and its members are released from any and all responsibilities in regards to decisions made during the time of an EMERGENCY. In the event that I cannot be reached, an adult member from Trinity Canadian Reformed Church has the authority to make an EMERGENCY DECISION, based only on the advice of the doctor in charge, to administer treatment if necessary.
